The LAM application form is usually printed as a double
sided A4 colour document, folded to A5.
These two .jpg images show the inside and outside.
The actual form is on the outside back cover, so you only need
print that single sheet.
But PLEASE READ ALL THE NOTES AND EXPLANATIONS, before you apply for
membership.
